CHPW used an outdated enrollment form that lacked good user experience. When I saw it, I volunteered to redesign the form, with a focus on how the user would interact with the form. My role was to create the UX and UI for this piece, scrapping the old form and completely redesigning it. This form was launched and is used to generate leads and enroll new members. As these forms were being used by our older, medicare population special consideration was given to older users, who might be interacting with the forms on their phones. We made sure to make the form fields and tyopgraphy larger to help these users engage completely with the experience.
